As the move on coordinator you’ll be responsible for a designated case load, providing direct support and coordinating their move on from the service in to long-term, sustainable tenancies in the local community. You’ll liaise with other support staff, the Deputy Manager and Project Manager to ensure we’re providing a wrap-around approach to support which enables our clients to develop new skills and make necessary changes to their lives. You will also ensure the move on programme is coordinated with other relevant stakeholders with a multi-disciplinary approach;

This role calls for some with a can-do, positive attitude and a proven track record in providing direct support to people with complex needs and chaotic lives. The role will be fast paced, with a number of competing demands so we’re seeking candidates who are able to effectively prioritise their workload and self-manage their schedule.
